Fifth option needed: NOT YET - but I need to explore! 

I changed from Eon with very complicated not easy to understand bills to Co-operative Energy in August 2011 and am very satisfied.  I read the meters on the 17th of each month and go on their website to send the meter readings and pay by monthly direct debit and get billed each quarter.  I also  get a dividend twice a year and also get £25 off the bill for every new household I introduce.  There is one tariff for each region and you pay a monthly standing order.  Co-op electricity prices go down by 2% on December 21st and at the moment both gas and electricity prices are fixed at least till mid- April 2013.  My bills are down from £900 to £768 per annum so I am very happy.

The new supplier does all the donkey work of changing and it is very easy to do so.
